The mc has a communication disorder and I think the most unrealistic part of the drama is how anyone can bear interacting with him or even understanding what he's trying to say unless it's an alternate universe where everyone has telepathy
He feels like one of these mute self insert nameless protags from japanese visual novels or harem anime where both fls and his bro keep doing all the talking and he just provides one word answers that somehow push the conversation forward. The fact that he has exactly one facial expression doesn't help either.
Exactly, the science of the drama is very off. However, I was willing to accept some things. I was willing to accept the concept of ruthless ultra charismatic psychopath serial killer straight from 1941 book "The mask of sanity", as it's quite popular in fiction, and from the beginning I labelled this drama as retro sci fi. Brain surgery and genetics only added to the retro sci fi feel, and it was fine.
But the mistakes in criminal investigation weren't fine. To add to the things you mentioned, nobody seems to know what "profiling" is and no one tried to create a psychological profile of a killer beyond occasionally screaming "it's not a human, it's a monster!". Maybe lack of profiling in 1990's would make sense, but in 2019 it's just criminal in itself.
Obviously this made me root for serial killer and not for police, so when he started getting repentant I decided to drop this one.

I predicted two different kids after episode 1, predicted the mc being headhunters son after episode 2, but whether he or yohan was the serial killer, I was actually quite puzzled until the reveal in episode 14. Mostly because I couldn't figure the "howdunnit" for some kills, like it didn't fit for example how he was able to make it in time to kill the priest and record it if he was supposedly in metro station watching camera recordings at the time. Of course, he was from the beginning the main suspect in the prison (attempted) murder, and I was surprised the police didn't consider him at all.

saiko-pasu! I did laugh every time they said it, and the premise of being able to tell who is a saiko-pasu and who isn't based on genes is ridiculous as well. Seems like the understanding of mental health presented here (and many other k-dramas dealing with the subject) is based on 1941 book "mask of sanity" and other such outdated notions.

Same feeling here, it makes it seem like entire korea is just one town where everyone knows each other and every other person is a psychopath. Still, this is just a style of fiction it is. Some fictions aim to be realistic, some are like gameboards with every character being a chess piece and every scene and event being relevant and connected to others (even beyond what is realistic).
