IMO the childishness is not required physically. The character is supposed to be an adult and doesn't need to look childish at all (on the contrary). It's his mental state that's childish only not his physique. Also, regarding his cosmestic surgery, I think everyone should stop judging it. That's his business. His body, his choice.
