For Khem, it was love at first sight. It can be seen on the slow motion scene when they first met. For Peem, it started when they were young and developed as they interacted while Khem was staying with him. That is why Peem's grandfather made him promise not to meddle in Khem's karma, because the grandpa knew he has something for Khem.

For Peem, he has been watching his grandfather heal/help Khem. So he knows him, the saving from the dog thing was not the only time Peem have seen Khem. When they became adults, he just didn't know it was Khem at first because they were kids back then and only recognized him through the necklace. I'm not even sure if you really watched the series or are we watching different ones? I'm not sure why you missed those flashback details.
