In ep 4 Makiko is actually being used as a tool of temptation to clearly show the character differences betwen Junichi and Keisuki, one is a great character not cheating neither his gf nor his first love, while one is evil, cheating on both his current and ex wives. With Naota occuping minimal screen time he probably won't be the main culprit of this drama, while Keisuki is likely now. I can't recall the exact details of what happening 23 years ago and Keisuki probably cannot be excluded from being the possible killer too. Might be the brother of Naota knew some secret of Keisuki causing his death.? In defence of Makiko now knowing her loved one no longer available she is within her right to do anything with other single man, as long as not cheating.

I can only think of 1 possibility that Junichi likely the killer of the rober 23 years ago (not a punishable crime now but still a crime, although morally probably right), which caused him to put a stone wall surrounding him, especially long relationships with women. Basically agreed totally with what you observed, but with the long wait for next episode just add a bit more of my opinions as this is my current favourite drama. Ep 4 is short only 45 minutes long and it spent most time in the character description of Keisuki, Makiko, and a bit of Junichi and his gf, that's kind of minimising the probability that they are the killer of the shop owner. The quadruple love relationships is all laid bare here too. Makiko's getting physical with her ex is a kind of reaction to her huge disappointment that her chance of a future with Junichi is dashed totally, and imaging what Junichi and his gf would be doing living together. But with 6 episodes to come, this likely would setup the stage later for the 2 leads to get closer considering the drama description with the detective ml meeting his first love in a criminal case ( but this need some dramatic events for this to be possible). But as a jdrama, ending is never predictable.
