People were trapped inside and died there, which could explain why they wore masks. There's even that scene where Yeli drags a body while wearing a mask, which makes me think the plague was a huge part of what happened.
The second is that it was a massacre. During the lockdown, something could have gone terribly wrong and someone decided the Lishan people needed to be wiped out. Maybe it was ordered by someone powerful, like the Empress Dowager
