i agree that communication is a big problem here. the explanation that I can think of why ML did not worry about her who has cancer is because of communication, fl tough behavior, and thinking about money, the 1 billion dollar club, and her family will take anything from her.
her tough behavior can be a reason why he did not sympathize with her when she dying but hugged her tight when she said she was scared in episode 5.
this communication problem already happened before when she miscarried and decisively got rid of any furniture for their future baby and did business talk without telling her husband what happened first and felt sorrow. her acting tough even in front of her husband is a problem(her husband kinda believes it) that makes him choose to have a separate room and their relationship deteriorates.
her tough appearance, and thinking about money and ownership did not make him empathy only made him feel morally wrong to be happy when his wife dying.
BUT I still think their way of making it funny by making him unconsciously happy is too far. Relief and worry are still better for me than being happy.

the different betwen JH knowing his brother sexual orientation and not is big. arange marriage its just make two people who in luke warm/cold relationship becoming husband and wife, on the other hand forcing a gay people to married an opposite sex is different matter.
