I think they killed off Tilan bcs she's a woman who keeps her promise. She promised to never let the emperor be lonely, that she will be with him in life and death. Even if she survives, she would give the regent to Haishi while she focuses on raising her son. The emperor hasn't write an edict to make her an empress, only for their baby to be the next emperor. The emperor will announce the empress position later when she safely gives birth and healthy again.

I knew this would most likely have a sad ending, but that was unnecessary. If they already made it different from the novel, why not make a happy ending for everyone? The ending is not satisfying. One couple dead, while one couple have to choose duty over love, and it's an open ending as well.
