As I see it, no, Hong Rim did not love the king, but he did care for him. Hong Rim didn't know any other kind of relationship than the one with the king, because he was so young when the king began loving him, so in the beginning Hong Rim thought he loved the king (which is why he found it difficult the first time to have sex with the queen). However, he did care for the king, because the king was always on his side and took care of Hong Rim.

I really like your theory on the scars, been struggling to figure it out myself! However, according to the plot twist in ep 12 at the end when it is revealed that the grandmother comes from ROK (that’s how I understand it), then shouldn’t she have a scar as well? Or else her doppelgänger in the kingdom would still be alive??
