It's not a harem drama. He stays true to his wife (the FL) until the end although I do remember there was some crazy girl who was from a rich/influential family who had a crush on the ML but I think it was resolved well and didn't bother me much. I am like you, I am no fan of harem dramas either.

So in the beginning the ML (Peter Ho's character)dates the 2nd FL (Ye Lin)because she has the same kind of eyes as the FL (Ye Lin) who is the girl who he has a crush on since she saved him. They break up because he finds out he doesn't really like the 2nd FL and when he meets the FL again he falls in love with her. The 2nd FL still loves him though and does everything to get him back. The 2nd ML the FL's fiance, later husband, later ex husband is just human trash. After struggling at work, he decides to find himself a rich heiress (the 2nd FL ) and schemes with her.
The 2nd male lead, Zhao Yuan Qing, the character played by Steven Jiang. He liked her through practically the entire drama but she doesn't like him. The forced marriage bit only lasts for a few episodes though thankfully. You don't realize that they were biological siblings until halfway through the drama (unless you've read the MDL synopsis) so it's not too bad, but the few episodes when you realize they are biological siblings and the Emperor and consort selfishly force her to marry Zhao Yuan Qing was pretty angsty.
A little, like a hint of it, I wouldn't really call it a romance

#1 In the book, it was pretty brutal, she was gang raped and FXS took a video of it that was how he managed to blackmail her into agreeing to meet those sadists. But I think he just threatened her with her fiance and parents in the drama. #2 & #3 yes rape happens in both but can't remember which one it was where he pimps her out and then rapes her afterwards, but they changed it into FXS messing up her fringe instead, which...I actually prefer. It's also the reason why I'm scared to read the manhua/webtoon as I'm told it goes into more graphic detail, but in the novel, it just tells you that's what happened without going into much detail (thankfully)

Two? You mean the one in the car and the hotel room right (I've only watched the first 6 or so) ? Yeah they're roughly the same except Xiao Wu doesn't appear yet so FXS is the one watching the sadist beat her up and rape her and afterwards he ends up raping her himself. She's doing everything she can to kill him at this point in the book so you could say the main plot is similar except there is much more Xiao Wu in the drama compared to in the novel.

Ah ok, that's what I remember from the novel (not the manhua which is still ongoing I think) maybe it's a little different in the drama. Feng Xiao Sheng does fall in love with Ke Ying in his own twisted way, but Ke Ying doesn't really love him although there are hints that her feelings changed a little after she became pregnant with FXS's kid.

He doesn't have surgery as Ke Ying recognizes him straight away and tells him at that creepy red dinner place that he looks exactly like the leader of the criminals who assaulted her.
Major spoilers: In the book Feng Xiao Sheng was the illegitimate son of the boss of the Feng mafia & conglomerate and because his half brother the legitimate son of the boss of the Feng mafia died around the time of Feng Ying's assault, the mafia boss faked Feng Xiao Sheng's death and gave him a new identity as one of the senior managing role in his company as he's now his only heir. So he's back for revenge for the fact that Ke Ying caused him to be arrested/hunted down by the police etc. He's supposed to be twisted because of his childhood and also he's the worst ML in chinese novel history.

From the trailers and the ED song at the end it looks like they've kept the fiance in and I think from the scenes and stuff I've seen he's still the love interest. Don't think that there is any romance between Xiao Wu and Ke Ying at least there are no romantic scenes of them together despite the fact he's the ML.
