I believe she knew his identity because they tried to dress like him in the beginning to lead the troops away. They replicated his attire knowing he’s the only one that dresses like that but he did have a mask on.

When people go through deep trauma some will take on two personalities. It’s a mental issue derived from not knowing how to process their thoughts and feelings properly. I think that’s what they are trying to display here. It’s not really a spirit per say. I guess they are trying to show when his split personality started. Hopefully in the end we will see the spirit leave him.
