Which one is higher in ranking between them usually depends on which family and supporter are more powerful. Both the queen and the queen dowager are daughters from a certain clan. and the clan is related to a certain faction. And it all depends on which faction is more dominating the government.
Even tho she has no king husband to support her, the queen dowager can be stronger politically. In Joseon history, even there's a case where she could be stronger than the ruling king himself. If you ever watched Mr. Queen, king Cheoljeong had no political power against the grand queen. And in reality he gained the power after her death.

The queen dowager's character here is a little complex. Imo she doesn't entirely evil. There's still a humanity left in her so far. She's just heartbroken, blinded by jealousy, misunderstood the situation and misguided. However that doesn't justify her decision.
She's still wrong. and if we look at why she did what she did, we know that at first she wavered to do the witch thing. At first, she kinda regretted it after that, she ever told the shaman she didn't want to do it again. She wasn't born as a mean person, but the situation built that character. She's emotionally weak that she's easy getting used.

After closely watching the intro, it looks like the 8-feet-tall ghost is the main ghost of the show. Maybe there'd be other ghosts presented each episode. But everything is tied to him.
