In most of Asia, the medical degree is conferred at the undergraduate level. In China, it's called MBBS.
So it's entirely possible that the ML received his bachelor's & medical degrees concurrently. Not sure how surgical training works, though.
And even in the US, there are similar programs in which you're admitted directly into medical school straight out of high school. Brown's PLME is a prototypical example.

Why are people rating episodes that haven't even come out yet? More importantly, why does MDL allow this nonsense?
It's pretty simple: $$$. Controversy sells. The more ridiculous the rating system, the bigger clickbait MDL becomes. More clicks, more money.
Good old fashioned American capitalism at work. A complete scam, but an effective one.
