Review showing up under someone else’s account

This only occurs for this title through my phone.  My laptop shows it with my information.  Even the link for the review on my phone is for the other person’s page.  Because all my reviews prior to the name change cause problems I’ve been trying to annotate them in the notes section but this was has an incorrect address  mobile-wise.
