I commented earlier as well, it looks like his eyes are drawn on painting by brush or pen something. He doesn't have caruncular (or it is deep inside), and his upper limb of eyes is drawn downwards and is connected with skin. That shape of eye is rare.

It's all because we can't tell what Do Hyun Soo is thinking. LJG did a fab job playing a character with anti-social disorder. When he is in any kind of emotion, happy, sad, or furious, it is very fascinating for me to guess and probe deep into his face to know, is he being real or fake, that game of guessing fake and real, keeps us on edge of our seats. But we know his leash is her love, thats where he ends. And thats why we enjoy this show. No matter what crumbles their life, she will never leave him and he will never be able to leave her.

She believed him, but as far as her safety and reputation is concerned, he will never let her run with him. thats why he cuffed her, so she can not be doubted or in danger. Thats love.

It was the life she would have lived if her husband wasn't killed.
So in her final stage of her life, she was diagnosed with Alzheimer's. And she created a story line where she was finally able to save her husband from a poor and tragic life in her imagination, as she wasn't able to save her in real life.
It was a healing process for her, as the guilt and pain she was bearing for 50 years, she finally let go so that when she dies, she meets her husband without guilt.
Thats why, she accepted her fate, gave up on a watch and finally reunited with her husband in last scene.
Drama has an elements of fantasy, but does not involves any magic or fiction.

So when she is diagnosed with Alzheimer's, she thought she could save her husband, as justice was never served so in her mind she became 24 years Hye-Ja and created the love story and life she would have lived if her husband hadn't taken away....
