No need for confusion. FL only saw the doctor as a friend with whom she can confide in. She could not talk about her daughter's death with anyone else, not even ML. So ML found the doctor so that she can walk out of her grief. Pity though, the doctor has no ethics and fell in love with his patient who is married and had kids. When he confessed his feelings for her, she flatly refused him, saying that she will not continue their sessions if he is bent on pursuing her. The doctor agreed to back off, but he actually didn't and almost took advantage of her in one of her weak moments during their sessions.

Novel's main story ends when ML is still in his early 30s and FL is still in her mid to late 20s. The extra chapters, there are 7 of them, explore our leads having kids (first time pregnant, they have a son. second time, twins, again sons), going though a miscarriage (losing a much awaited daughter), moving on from it, to living to the old age when they die. I think it makes sense to show them dying. It brings a closure to Han Ting's declaration, which is also my favorite line of the entire novel, "The love I believe in probably lasts until the end of life. When I look back, I will have a definitive conclusion." All I will say is, I am always a crying mess whenever I read the last extra chapter. And I am someone who rarely cries reading fiction.
