If you read carefully the description, it says that they meet in a hospice. The doctors in a hospice take care of terminally ill patients.

Great article! I am currently watching SFD after putting it on hold for a while and it is surely full of amazing characters.
I didn't expect to see Ji Chang Wook as Ta Hwan here, but I completely agree with you. Even if his character was annoying and immature most of the time, he was a person who was lonely and needed love. He felt like everyone has abandoned him, and he went crazy because of it. He didn't fit for emperor and that was the great part of his character, because as we know emperors in China are crowned at a very young age, and they know nothing of their responsibility.
