I know that they didn't use CGI :) I have seen the interview with the cast, plus the writer himself interviews. It's basically a fake body and intestines and the cast had to learn some parts of the surgery, plus real doctors and nurses were on the set as well adding to the realness, which I can only applaud for. I am also happy that the main lead actor did mention the OR contamination scene, and that it also bugged him. In reality, such scene could never happen. If someone came and contaminated my OR as a scrub nurse I would redo their vertebrae for sure.

Yeah, the nuance was lost, as in English it made him look like he is asking God (sin) for help, but in Korean it was more savage-like basically him being confident and saying to God that the God should chill and trust him, in a way portraying himself as someone great.
