Another thing to note besides what some the other comment says is Japanese dramas still tend to be heavily rely on fansubs rather than official subs released by a company. Its changing slowly but some broadcasters aren't as interested in licensing their content abroad. In additional, a lot of the "historical" dramas or period pieces can be quite long dramas. Notable examples include the "Taiga" dramas from NHK which can be around 50 episodes. Its a lot of work to put in to make subs for 50 episodes especially when a typical Japanese drama might only be 6-12 episodes. Another thing about subs if they do exist is fansubbers do not appreciate all the illegal streaming sites making money by taking their hard-work and posting it on their sites as their own. Depending on the subber, they might only be released as a hardsub or released on a site with restrictions.

I belive its mainly because when news about casting and the drama first comes out, the English Title is more of a literal translation of Korean title. Later on, they always change the English title to something else. How they choose to do so though, beats me.

Most production companies won't admit this openly. Reality shows always have some kind of "steering" going on behind the scenes to make sure they get enough footage or so the production company gets what they want. So while not "scripted" as in they have a line by line script, they might be recommended to do certain things.
