As I stated before, the titles in this drama may not necessarily related to the real history. They are referenced, but the production team still change several things like the King's name, events, and etc. This is a romcom fantasy drama, it's not supposed to be historically accurate like Queen For 7 Days (also about Yeonsanggun).
I understand the titles are confusing. I think on Netflix, the subtitle for her is Concubine if I'm not mistaken. Maybe they call her that because she is highly favored? In this drama, she's not married to the King. She's merely a woman who gained high favor from the King.
We don't know exactly yet how she ended up in gaining the King's favor. Maybe they will reveal that in later episode.

I also made a mistake about consort titles. They have like Primary Consort, Secondary Consort and etc. But in history, Jang Nok Su begins her title as Concubine and she gradually rises up. She only get called Royal Consort during Yeonsanggun's reign prime. She didn't gain the Royal Consort title in day 1.

The production team for this drama has put disclaimers that they changed several things from web novel (the original work) and this is a fantasy drama. I mean yes, Yi Heon + several characters and some events are based on real tyrant king, but the timeline of his reign in this drama is unknown because of the changes made. So we may not able to tell it correctly. Better to see him as a new character rather than limiting him as the real king in history.
