The main difference is actually mostly the historical facts in the drama. Because Emperor Kangxi and the fight for succession with his sons were real (you can search them up), it made the plot run really smooth. The most fiction thing about this drama is the mc because she doesn't exist. The Korean version is definitely more eye candy with IU and the historical part wasn't as realistic. I still love watching IU in it though. Both versions are good, but this one is more heartfelt and deep.
