And there are situations when you should visit hospital and even be hospitalized with diarrhea! Some of them are person-specific. In Seng's case, it looks like he has his own reasons to avoid the food, be it physical or mental.
That said, BLs tend to understate/overstate medical stuff all the time. One example of a BL character ending up in hospital due to diarrhea is Song (Tee) from TMoTB.
