After rewatching ep 21 with a different sub, I finally understood the confusing subtitle on my first watch. The lady/madam the soldiers were referring to is actually QQ. And that what's QM meant when he said that 'that woman is nothing' and then had her bound. And it also connects to a scene in the later episode where he said he can't let others know she's his weakness. So he's really trying to keep appearances that QQ is not that important to him.
There's a very logical reason for this from his pov, but it contains a lot of spoilers.

Yes, he is really twisted. He grew up traumatized, no one showed him how to love or be compassionate. His supporters only cares for his bloodline and wants to control him. Imagine being a child fed with thoughts of revenge, hate and propaganda. Add the fact that he was in physical pain from all the skin grafts. He's actually a pitiful character. Letting BaoEr live is instant death to himself (atleast from his pov). It would mean all the suffering he endured are for nothing. Plus the circumstances how BaoEr came to be is something he absolutely despised. I do think if their child was a girl, he would actually care for it.

QiMin is actually the legitimate heir to the throne. His Mom burned his face so they can swap him with the real Sui kid and to keep him safe. This also the reason why he's got a bunch of loyal followers who are waiting for him to take revenge and seize the throne. They don't truly care for him but more for his bloodline. Because he was sickly, they fear he might die soon, so they drugged him and forced to breed with QQ. The child's existence is literally a threat to his own life. Since now there is a younger, healthier boy that they can install as a puppet Emperor. They have no use for QM anymore and would eliminate him anytime soon.
