See you have to remember to actually think about those things. This isn't a discussion about writing a story for an audience but rather writing a real person. Also just to note I'm not 100% caught up "yet" so this is based off what I have seen. But the ML was shown to have a diary where he records his thoughts more in "real time" than is "appointments". When "most" people are recalling things from what I understand there are emotions that come up and help trigger those memories. When you are lacking an emotional anchor to a memory unless something is happening RIGHT THEN it doesn't matter. (Now whether this is actually accurate or not is debatable, I'm just trying to describe what it "feels" like to try and recall things and most of the time it's basically the same as reciting what the weather was as it was what I was doing.)
Now as for his SYMPTOMS. You have to remember that without an emotional response he's NOT thinking about those things so no, they wouldn't be brought up in therapy. Is that wrong? Personally I've meant to create a list of things to bring up and have that to go through but again, that requires remembering to do it and the brain often blocks out what it DOESN'T want to talk about, hiding it from you.
Finally, you do mention the wrist pain as being brought up in the session. If you rewatch it, he brought it up because it was ACTIVELY happening at the time. Through out the entire scene he had been rubbing his wrist in pain and this triggered "bring this up". So if his heart rate were to increase suddenly WHILE he's talking in a session this would ALSO open up a discussion (and now I'm actually wondering if they will do it this way). With this too I have a literal case in point actually... I was in my medication appointment last month when MY watch showed that my heart rate was increasing. This triggered me to actively bring it up AND recall that there had been a few times over the past few months this has been happening more frequently. Have I had appointments since it started, yes, I just didn't have a "trigger" to make me recall the fact.

Sure I'm doing them through Zoom and not going over to the guys office/house whatever it is and just walking in... THAT'S a bit out there, but the "this is what has happened since I saw you" talking is very similar. He usually starts the session with "how are you" and I'm like "I'm alive" or "I'm awake" or "I'm tired" and then then tries to coax a bit more. Relates his own stories sometimes, explains what the situation might actually be if "too" logical etc. As for his reports 🤷♀️ If you've seen someone for years I'm sure sometimes you just wite a quick update to remind yourself of something rather than spend an hour writing a detailed session summary report. Especially since this is China and they don't have to worry about health insurance companies sticking their noses into their reports I'd imagine.

Idk if you're still watching this or not but in case you are there is absolutely a case to be made in "delulu land" as some people called it for watching the last episodes and considering it a HEA. Yes, in this mindset everyone in the group aside from the ML/FL will still die but the theory is that "somehow" the ML faked his death because of his special armor and the magic pills they mentioned having earlier on in the series. Then, the FL ALSO fakes her death again and they dissappear. The last scene of the town and the kid with the teacher is their kid "proving" this theory. 😅.

So the time on foot for her couldnt have taken a year though because the Prince says its been almost a year since he last saw Zhang Jinran and that time accounts for at LEAST everything that has happened during the course of the show plus the original travel time to get from Prince to where we first see him on the boat. 😅
Honestly I love that he is willing to take on BOTH main and support roles because it means we get to see him in more dramas so often without being "as" worried about full burnout.

I saw you skipped around a bit trying to watch it. I'm not sure if you saw it or not but the stakes did change once she left the "beginner's stage" or whatever they called it. So now she no longer has unlimited respawns and the stakes are slightly higher. Like all shows I think there is a bit of a take-off curve which for this one would be the 5 eps they released. They have now set-up character personalities and motivations and introduced an antagonist so subsequent episodes should have them "doing" something like you said.
That said, because Esther Yu basically sounded and acted the same in the modern world I can't really see her voice and mannerisms changing much over the course of the show. After all there doesn't "seem" to be a plot so far where she would get controlled by the game/novel and have to sound/act different. IMO it's better to not be invested early on if it doesn't grab you than to keep coming back and forcing yourself just because you see other people liking the show.
