You give access to a sick person who is not awake and aware of and responding to his environment, only to people you know that sick person would trust without question, not some random person like Pai, who Sky has said he doesn't want around. Pai is just a rando to Sky, even though there's some interest that goes both ways. This is a horrendous breach of trust by Rain on its own, made worse because the audience knows Sky was assaulted.

Otherwise, you'll understand their dynamic better if you know that it's got elements of Dom/sub in it. Sometimes MAME makes a mess of things though. In the 2nd episode Phayu asks Rain to tell him to let Rain go, to which Rain says "No" as in I will not ask to be let go, which is consent to what happens next. That's basically a D/s scene, and you might miss that, if you don't know anything about Doms and subs.
My final verdict is that I like it, and it's also sweet and emotional, not just WTF are you serious right now, kinda show.

Yes, Sky was pressured, it was obvious, even more so in the book. He was like "Fine, take what you want then, I can deal with having sex with you." He asked for the condom and what not, because he wanted to just get it done already. Sky was ambivalent, like, "Whatever lets get this over with, lets get to the main event. I don't have to stay too long and I'm not giving you my name or number." That was the vibe of that scene.
