Plus Princess had told her mother that she saw ML as her brother only, this conversation was even shown to us in the drama when her mother told her to marry Li Huai An, because princess rejected ML, cause she saw him as her brother only. Qi Min has been weak due to the fire in his childhood. Plus the multiple skin grafting techniques made him weaker. He isn't healthy like his brother or others, hence the white hair. But he is not currently at the "dying stage". The coughing in front of his brother, father, other people are him pretending to be weak so that they think he is not a threat.
