You've made lots of interesting observations. RTZ definitely has a long held grudge against Tou Tou due to the food delivery incident. The fact that his father dotes on TT has made him even more resentful of her. I believe he sees her as the intruder disturbing the family dynamic, where there should only be Meng Meng. Getting TT drunk was his revenge (disproportionate retaliation), perhaps he also hoped that she would be kicked out of the house.
As you said, RTZ has a lot of pent-up anger, frustration and resentment. I understand his family background is both a blessing and curse, at the very least heavily restricting and putting great pressure on him. Maybe that is why he cannot stand TT and her carefree attitude. She is free to be and do what she wants to an extent that he never will be.
The situation with MM is handled very poorly. She is emotionally and mentally unstable, although her state suggests to me there might be more trauma affecting her than the ectopic pregnancy and its aftermath.
If the Ren family is trying to overcompensate and thus strike a balance to help MM heal emotionally, RTZ should be prepared to fulfill this role for however long MM needs him - possibly for life. It would be very cruel to make someone the centre of your universe, then take away that attention and care. I am not convinced RTZ has romantic feelings for MM at this point. Unfortunately, he is also creating a lot of expectations in Meng's heart and mind. What next? Is he willing to care for her like this long term? In what capacity - boyfriend or future husband? The doctor-patient relationship flew out the window a long time ago.
To me, how the Ren family decided to make amends to MM is unethical and can be dangerous for everyone involved. I am not surprised by RTZ as he is clearly irresponsible. But his parents are experienced and should know this may cause more harm than good.
I know we already formed an anti-fan club for RTZ... But I wanted to add something that looked unbelievable to me at that time. Remember when the girl in the wheelchair and her mom (sorry forgot their names) met face to face with the lady who caused the girl's accident? The mom starts hitting the lady and lets go of her daughter's wheelchair.
In that scene we have 2 situations that needed immediate help - stopping the assault (it didn't look to me that the lady was fighting back) and making sure the daughter was ok. Yet RTZ only cares about MM, who was huddled in a corner.
The issue here is that he completely ignored 2 critical even dangerous situations and prioritized someone who was scared but far from any potential danger. Fortunately, someone grabbed the wheelchair and Tou Tou stepped between the women. Again while RTZ stood there watching.
Others mentioned below that script writers went overboard when creating RTZ's character, who either acts like an irresponsible teenager or shows little professionalism as a doctor. For me, it will be very difficult to redeem RTZ - he is so disappointing both as a professional and decent person. From the very beginning, he shows this innate arrogance, combined with lack of compassion and consideration for other people's situation. It's most obvious when he interacts with Tou Tou. But even with his long time friend ZLQ, being aware the guy has a poor family and urgently needs a paying job, RTZ asks him to not go to a conference (potentially useful for connections or even a new job) and give his earned place to Meng Meng.
Don't get me started on the story around MM. I cannot imagine what the script writers were thinking. How is that bizarre living situation/treatment helping her to heal? It's problematic and yes potentially dangerous on so many levels for MM and the Ren family.
And nobody asks themselves whether the trauma is deeper than the ectopic pregnancy. It is said MM is young - then was the sex consensual? Even if she is legally an adult, she could have been a victim of sexual abuse. Bottom line, she needs professional help including therapy - and RTZ could accompany her as a friend to offer emotional support since he wants to make amends.
However, I doubt he will apologize, let alone make any amends to Tou Tou. Let's not forget that MM is very pretty and the vulnerable type that triggers the knight in shining armour complex. He was barely civil when they first met in the supermarket, yet now he radically changed his attitude.

Maybe the writers' point is to show that coming from a priviledged and loving family background, with plenty of education and other resources has not shaped Tian Zhen into a professional adult, let alone a decent human being. He benefited both from good genes (nature) and refined upbringing (nurture).
Meanwhile, Tou Tou supposedly has the great genes from her mother and little informal or formal education. Yet she has much stronger moral principles and compassion that guide her behaviour. In time she will probably surpass Tian Zhen in skill.

I think the root of RTZ's behaviour is that his mother has consistently downplayed or covered up his mistakes from childhood until now. He has yet to face the consequences of his poor choices. That is why he smiles almost jokingly to his mother when saying he regrets what happened with Tou Tou and will not do it again.

No matter how much he dislikes Tou Tou, how could he purposefully get her drunk then leave her there alone? Anything could have happened to her. The scenarios you described are certainly possible. I would prefer for the drama not to go the route of XM dying. Not only would it be hard for us viewers, it would also shift the focus too much on TZ, his struggle to recover etc. I find that there much more interesting issues and relationships for the drama to explore.

From that brief appearance, you could tell already that Xu Meng has a vulnerable air around her that tugs at heart strings and makes others feel like protecting her.
On the other hand, Tou Tou is fierce, used to survive on her own, quite aggressive at times or turns what she said into a joke especially when she feels emotionally raw. Tian Zhen dislikes, doesn't want to and/or cannot understand her, since he comes from a privileged and loving family. Of course, he bears his share of difficulties. But their backgrounds have shaped them into fundamentally different people.
More importantly, there is a high chance Tian Zhen will resent Tou Tou even more once her relationship with his dad deepens. Many scenes suggest Ren Xin Zheng will be more like a father than a mentor to her. He already dotes a lot on Tou Tou and openly favours her, while towards his son he can be very strict and sometimes too harsh.
Obviously, it is all speculation on my part. I am curious though how the relationships will unfold.
