Looking back on it, this was terrible. The female lead is super naive, there was really no reason to even like him tbh, the male lead treats her horrible pretty consistently. Most of the cast was also very annoying.

Yes. He granted her no pain and an extra wish for the exchange of her wishing for the world to end, that's the contract they went into when he saved her from the white truck of death. But if she were to break the contract and to not wish for the world to end then he'll kill the person she loves the most (her brother at the moment). There is always a penalty for breaking a contract.
