How they transfer the tattoo to each other is beyond me. It's happened twice now and there were only 2 thing common…
The first time the tattoo transferred, it was because of Do Hee's desperation to save GuWon even though she might die. Plus it happened in her element - under the water. The second time the tattoo transferred, it was because of Gu Won's desperation to save Do Hee even though he might die. And this time, in his element - surrounded by burning fire. And then there's also the full moon variable. It wasn't just a random save each other kind of thing - because they have already saved each other a bunch of times. There were very specific rules to hit. On both times the tattoo transferred, the rules were fulfilled.

Gu Won started to really fall when Do Hee rescued him at the end of ep 4. That "I'm my bodyguard's bodyguard" planted the seed. The way he cannot believe at how this seemingly "insignificant and weak" human comes running to rescue him (and very adorably at that). That was the first real moment.
Pretty much quickly escalated after that with the threat of some other man becoming Do Hee's husband. That "Marry or divorce whoever you want. Just don't show me that kind of face." says a lot more than the throwaway indifference he was oh so trying hard to insist on.
His feelings were pretty much sealed at the end of ep 5/beg of ep 6 when he all but flies towards Do Hee's apartment. That "Do Do Hee please" as he rushed to save her... ugh I felt that! But yes, pretty much this is the real turning point as he realized he wouldn't know what to do with himself if he loses this "insignificant and weak" human. Hence why he easily jumps into a marriage he doesn't believe in.
At this point, nothing else matters beside keeping Do Hee safe. He didn't just say "I don't want you to die". In Gu Won speaks, it pretty much meant "I would never let something that horrible happen to you ever again". And Do Hee heard. That's why she chose him.
After that, Gu Won's feelings pretty much bloomed to the point that even in his dying breath, it is her wellbeing that he is worried about and it is her name that he speaks.
And of course, at the end of ep 8, we come full circle. "Even if my feelings for you make me insignificant and weak, you are my fate that I cannot defy". He gladly and willingly steps into and embraces the identity he had wholeheartedly abhor for this girl who has his whole heart.
